The Red Songbird Foundation is presenting a therapy scholarship honoring The Dolbeare Family.
Our hearts go out to the Dolbeare Family everyday, but especially this month.
The Red Songbird Foundation is grateful for our community and treatment professionals who believe in the work we do. We can’t think of a better way to honor the Dolbeare family this Holiday season and want to gift an opportunity for healing from grief through therapy and ongoing clinical support.
Cost Details:
Criteria: Client will need to be over the age of 18 and have an ability to clear an assessment. Extensive health issues may impact clinical appropriateness.
Criteria for Applicants:
✓ Male or Female (18 Years or Older)
✓ Financially unable to afford therapy
✓ Want and willingness to take direction towards a better life
To assess your needs, we need to collect protected health information (PHI) from you. To comply with the Health Insurance Portability and Accountability Act of 1996, more commonly known as HIPAA, we will treat your PHI as confidential information and not disclose it to anyone other than as necessary to provide you with products and services. However, please note and understand that this form is not HIPAA compliant and the information is sent via encrypted emails that are also not HIPAA compliant.
